Incrediable lightning picture!!
I found this picture on a website whilst looking for more pictures to put on my wall amazing id say there is atleast 200 strikes in that picture.
It was taken East of Sydney.

There's a similar photo in a photography book I have, taken with a 35mm film SLR with an exposure of 30 minutes at f/2.8 but that was on a beach with no other lights about.
For a brighter background you could try the trick for combining photos that I've used for star trails on a digital camera. In Paint Shop Pro use Image Arithmetic in Lightest pixel mode (other programs have similar commands). That way the background and constant lights stay the same.

ND filters, like stopping down too much, tend to make the thin stepped leaders invisible - something else I learned during the storm in January

Its possible that is one exposure only- if there was a time lapse the cars on the ground would show up as streaks and the building lights whould show evidence of smudging.
I'm more of the opinion that this has been doctored with photoshop where anything is possible.
